C3.2 R5: Perform and document a Best Possible Medication History (BPMH)

MP is a 16 year old girl who entered our adolescent program (4 weeks long) to help control her manic episode and to provide stabilization. A MedRec at discharge was provided to family, and the practitioner and for the chart. The MedRec was as follows:
